GENERAL ENROLMENT FORM - Face to Face Courses

To enroll in our courses please complete the form below

IMPORTANT!!!!

COURSES NOW CLOSE FOR BOOKING 2 WEEKS BEFORE

Please ensure you 
1. correctly write your FIRST AND LAST NAME. (this will be on your certificate)
2. correctly write your ADDRESS IN FULL (this is where we send your manual)
3. select the CORRECT COURSE AND DATE from the drop down box below.
4. for courses in Australia with PRACTICALS (eg Adv POP/SUI and Adv Pain) please see note for travelling physiotherapists in side column

PAYMENT

No Payment is required until your enrollment is confirmed by our admin team by email.

At that time a 20% Deposit is required within 2 weeks to finalise your acceptance into the course.

 - this 20% is NON-REFUNDABLE and covers administrative costs associated with the course.

Final Payment (remaining 80%) is generally due 1 month before the course start date
