GENERAL ENROLMENT FORM - Face to Face Courses

To enroll in our courses please complete the form below

IMPORTANT!!!!
correctly write your FIRST AND LAST NAME. (this will be on your certificate of attendance for PD)
Please ensure you select the correct course & date from the drop down box below.

PAYMENT

No Payment is required until your enrollment is confirmed by our admin team by email.

At that time a 20% Deposit is required within 2 weeks to finalise your acceptance into the course.

 - this 20% is NON-REFUNDABLE and covers administrative costs associated with the course.


Final Payment (remaining 80%) is generally due 1 month before the course start date, however during the COVID-19 crisis physiotherapists whose clinical practice / employment has been affected can request an extension to pay until after social distancing recommendations have been removed and employment circumstances return to normal.
